Age UK Reconnections Service (Worcestershire)

Aimed at tackling feelings of loneliness amongst older people by supporting them to connect with local communities through their own interests.

Tackling loneliness in such a personalised way is designed to enhance wellbeing for older people and reduce the use of more substantial health and care services.

Clients must be over 50 years of age

Clients cannot be in receipt of a Social Care Package or NHS package of Care or in the process of assessment for either package - (self-funders would be eligible)

Clients cannot have, or be in the process of assessment for, a diagnosis of Dementia

Clients must pay (or have paid on their behalf) Council Tax to a Worcestershire District Council.
